Annual Grooming/Boarding/Daycare Terms & Conditions

RESERVATIONS: Grooming/boarding reservations must be made in advance.  We cannot accommodate walk-in appointments.  If you have an urgent need, call the office before you come and we will check to see if there is a vacancy. During busy overnight boarding seasons, daycare space will be limited. Always call before bringing your pet for day boarding if you have not reserved a spot ahead of time.

HOURS OF OPERATION: I understand that Patterson Veterinary Hospital is open to the public between 7:30am and 6:00pm Monday - Friday. 1) Grooming drop off/pick up times are decided by the groomer but are always during our business hours. 2) Daycare and boarding pets can arrive and depart any time during these hours. 3) We are closed on Saturdays, Sundays and Major Holidays. . 4) We no longer offer weekend boarding services effective June 1, 2021.

CANCELLATIONS: If I have an existing grooming/boarding or daycare reservation but have a change of plans on a certain day, I will let Patterson Veterinary Hospital know at least 24 hours in advance so that a space can be made available for another pet. (No-shows may incur a cancellation fee or restrictions on future reservations since we are not able to fill those vacant spots with other pets at the last minute.)

VACCINATIONS/PREVENTATIVE CARE: I understand that grooming/boarding/daycare pets must have required vaccines unless advised otherwise by our veterinarian. The required vaccines for dogs are: Rabies, Distemper/Parvo (DAPP) and Bordetella (Canine Cough). Cats are required to have a current Rabies Vaccine. If these va
ccines were administered by another veterinarian, I agree to provide documentation or a name/number where these records can be obtained PRIOR to a grooming/boarding reservation. Any vaccines that are due or overdue at the time of boarding/grooming/daycare will be administered at our veterinarian's discretion and at my expense. I also agree to keep my pet protected by a flea/tick preventative and understand that he/she will be treated immediately if live fleas/ticks are found during grooming/boarding/daycare.

INFECTIOUS DISEASE CONTROL: Patterson Veterinary Hospital cleans and disinfects the grooming and kennel areas daily to prevent any illness from being passed from one pet to another. I agree to notify Patterson Veterinary Hospital of any symptoms my pet is or has recently experienced before bringing them for boarding/grooming/daycare services. Symptoms include but are not limited to: vomiting, diarrhea, coughing, sneezing, lethargy, behavioral or appetite changes, and rashes. I also agree to notify Patterson Veterinary Hospital if my pet has had an exposure to an infectious disease at dog parks, training classes etc. within the past two weeks including but not limited to kennel cough, canine influenza, parvovirus, etc. ***RABIES CONTROL: IF MY PET HAS BEEN INVOLVED IN AN ALTERCATION WITH ANOTHER ANIMAL (wild or domesticated), I WILL NOTIFY/PROVIDE NEEDED INFORMATION TO PATTERSON VETERINARY HOSPITAL IMMEDIATELY.***


HEALTH/MEDICAL PROBLEMS: I understand that grooming/boarding can be stressful for pets that have are not used being away from home or have had bad experiences. Stress can expose hidden conditions including allergies or aggravate a pre-existing one. If any concerns are noticed during the period which my pet(s) are in the care of Patterson Veterinary Hospital, the staff will try to notify me. In the best interest of my pet, I give Patterson Veterinary Hospital permission to provide immediate treatment for my pet(s) should it be deemed necessary. It is agreed that all expenses for veterinary care will be covered by me, the pet owner, upon signing this agreement.

 


EMERGENCY TREATMENT: If I do not inform the Hospital otherwise regarding measures to be taken, or if the state of the my pet(s) health reasonably demands quick action in the opinion of the veterinarian, the Hospital will administer medical treatment, anesthesia, and/or surgical treatment as needed, for which the owner is financially responsible, until the owner can be notified.

PHOTOGRAPHS: I understand that photographs and videos are taken of the hospital, pets, clients and staff on a regular basis for purposes including advertising by Patterson Veterinary Hospital. If I have given permission to allow the use of photography of my pet, I acknowledge that all such images, together with prints and copyrights, therein are the property of Patterson Veterinary Hospital.

BEHAVIORAL CONCERNS: I agree to inform Patterson Veterinary Hospital of ANY behavioral concerns my pet(s) has or has had in the past. This includes whether the pet will bite, has bitten in the past, is aggressive towards people and/or other pets/small animals. I understand that muzzles are used only when necessary and for the shortest period required for the safety of the groomer/kennel attendant. If my pet is new to grooming/boarding, I understand that extra patience and understanding is needed and will be provided for my pet(s).

DAMAGES/INJURIES: I understand that I am solely responsible for harm or damage caused by my pet(s) to persons or property of the owners, employees and invitees of Patterson Veterinary Hospital, or any other pets housed or visiting Patterson Veterinary Hospital while my pet(s) is/are attending Patterson Veterinary Hospital.


PAYMENT: I understand that I am responsible for grooming and/or boarding/dayboard fees and associated costs. Payment is due at discharge (for daycare, payment is due on the last day of each week that my pet is dayboarded). If someone else is picking my pet(s) up, I agree to prepay at admission or before discharge via phone payment. We accept Visa/MC, Amex, Discover, Care Credit, Cash and (Checks: established clients in good standing only). If someone else is picking up my pet(s) I agree to notify Patterson Veterinary Hospital staff beforehand.


ABANDONMENT: If I do not pick up my pet(s) within 5 days of the scheduled discharge date, the Hospital will assume the animal is abandoned.  If the animal is abandoned, the Hospital is authorized to remedy the abandonment as prescribed by law.  I further understand that abandonment DOES NOT release me of my financial obligation for services rendered, fees associated with abandonment, collections action, and/or legal services.
